To support the assessment, planning, implementation and evaluation of packages of care for children and young people with acute and chronic illness, complex disability or palliative care needs. To work in partnership with the children, their families and Carers with other professionals from Health, Social Services, Education and non statutory agencies to deliver care to meet the child and young persons assessed needs. To provide care that is flexible and responsive to the individual and meets the demands of the service as a whole. This will include working at Special Schools and support to paediatric clinics within Child Health.
To work with young people, with complex health needs, their families / carers and others within health care to promote and contribute to a seamless transition process between children and adult services.
