About the Role
We are a vibrant and forward-thinking multidisciplinary team dedicated to delivering exceptional care, support, and guidance to patients, their families, and our community. In this role, you will:
1. Play a pivotal part in shaping the future of Integrated Neighbourhood Team (INT) working.
2. Lead initiatives aimed at preventing hospital admissions and facilitating smoother, faster discharges.
3. Collaborate closely with our team leader to provide tailored care to a diverse range of patients with complex and non-complex needs.
