This role is for aregistered nurse with a qualification appropriate to the skills of a MentalHealth Practitioner. This role will be based at Castlegate & DerwentSurgery (1 Isel Road, Cockermouth, Cumbria, CA13 9HT) and Maryport HealthServices (Alneburgh House, Ewanrigg Road, Maryport, Cumbria, CA15 8EL)
The post-holder will beresponsible for providing comprehensive mental health specialist care withinthe organisation and is expected to develop, plan, coordinate and deliver theservice while working as part of the organisations multidisciplinary team.
The role is subject to a successful six month probation period. The salary is based on experience but will be offered equivalent to mid-top Band 7 (Agenda for Change, but the organisations do not follow Agenda for Change)
Main duties of the job
The Mental Health Practitioner will work as an autonomous practitioner and be responsible for the provision of assessments, interventions and education. They will have responsibility for managing a caseload of patients with a variety of mental health conditions and will be expected to collaborate with other healthcare professionals both internal to, and external from, the organisation. They will also be expected to hold a prescribing qualification and evidence their ability to prescribe within their professional competencies.
The post holder will have access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis.

Job responsibilities
The following are the coreresponsibilities of a Mental Health Practitioner in delivering healthservices, noting that there will be additional roles and expectations dependenton the qualifications and experiences.
a.Workas part of a multi-disciplinary team (MDT)
b.Providemental health advice, support, consultation, including acting as a firstpoint of contact in primary care for patients whose care needs are notsuitable for Talking Therapies services
c. Work with patients to:
Supportshared decision-making about self-management
Facilitateonward access to treatment services
d.Completecomprehensive risk assessments informing outcomes and positively managingrisk indicators and to demonstrate risk assessment and management skills, practicingsafely and according to individual patients needs
e.Toassess the needs of patients ensuring the provision of healthcare isappropriate, incorporating evidence-based practice
f.Todevelop, implement and embed health promotion and wellbeing programmes
g.Toprioritise health issues and intervene appropriately
h.Tosupport the team in dealing with clinical emergencies
i.Tosupport patients in the use of their prescribed medicines or over-the-countermedicines (within own scope of practice)
j.Todeliver opportunistic health promotion where appropriate
k.Workingclosely with Social Prescribing Link Workers to address patients housing,financial, vocational and other practical needs.
l.Toliaise with external services/agencies to ensure the patient is supportedappropriately (vulnerable patients etc.)
m. To delegate clinical responsibilitiesappropriately (ensuring safe practice and the task is within the scope ofpractice of the individual)
n.Tosupport the clinical team with all safeguarding matters, in accordance withlocal and national policies
o.Tounderstand practice and local policies for substance abuse and addictivebehaviour, referring patients appropriately
p.Providea range of therapeutic interventions for people experiencing acute mentalhealth problems
q.Communicateproactively and effectively with all colleagues across the MDT, attending andcontributing to meetings as required
r.Maintainaccurate and contemporaneous health records appropriate to the consultation,ensuring accurate completion of all necessary documentation associated withpatient healthcare and registration with the organization

v.Takethe lead role in undertaking comprehensive culturally appropriate,evidence-based initial health and social needs assessments, determining thespecific care needs of the individual and in the formulation, development,implementation and evaluation of individual and management treatment plans
w. Operate without the need for a formalreferral from GPs, including accepting some direct bookings where appropriate
x.Workwithin national and local protocols
y.Participatein formal training events promoting best practice in area of expertise
z.Contributeto the delivery of local policy, procedures and protocols to improveidentification, assessment, ongoing support for patients and their familycarers
aa. Support the delivery of QOF, incentiveschemes, and other quality or cost effectiveness initiatives enhancingservice delivery and patient care
bb. Recognise the boundaries of theirpractice and know when and to whom patients should be referred
cc. Be aware of duties andresponsibilities regarding current legislation and adhere to practicepolicies and procedures on safeguarding adults and safeguarding children
There may be, on occasion, a requirement tocarry out other tasks, this will be dependent upon factors such as workload andstaffing levels
Person Specification
Qualifications
* Holds a professional qualification at diploma or degree level in mental health nursing or equivalent occupational therapist qualification and has a valid registration with the governing body.
* Independent Non-Medical Prescribing qualification
* For an Advanced Practitioner, be educated to Masters degree level in the relevant area of expertise and has the capabilities of advanced clinical practice
Clinical Knowledge & Skills
* Understanding the importance of evidence-based practice
* Ability to promote best practice regarding mental health matters
* Ability to work within own scope of practice and understanding when to refer to GPs
* Ability to record accurate clinical notes
* Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email and face to face
* Good clinical system IT knowledge
* Broad knowledge of clinical governance
* Knowledge of public health issues in the local area
* Awareness of issues within the wider health arena
Experience
* Two years experience of post graduate work in a core profession following qualification
* Experience of working autonomously
* A clear understanding of child protection policy and procedures and a commitment to the safeguarding of children and vulnerable adults
