What you will do:
Work closely with patients, families, and multidisciplinary teams to ensure safe, timely, and effective hospital discharges.
Complete comprehensive assessments to identify patient needs and develop appropriate care and support plans.
Coordinate with community services, health professionals, and social care teams to ensure continuity of care post-discharge.
Advocate for patients' rights, ensuring their voices are heard and their best interests are upheld.
Maintain accurate, timely, and high-quality records in line with professional standards and statutory requirements.
Participate in case conferences, offering professional insight to improve patient outcomes.
Engage in regular supervision and continuous professional development to maintain high standards of practice.
