Collaborate with physicians, specialists, and other healthcare professionals to ensure seamless care delivery and accurate information transfer. Document patient interactions accurately and thoroughly in electronic health records (EHR) following institutional and regulatory guidelines. Qualifications Active and unencumbered Registered Nurse (RN) license in [Insert location/state as per role]. Bachelor’s degree in Nursing (preferred) or Associate’s degree in Nursing. Day-to-day Begin your day with a team huddle to align on patient priorities and overall care goals. Triage incoming calls or patient visits, determining the urgency of care and making recommendations for treatment or resources. Communicate with patients about symptoms, treatments, and care instructions in a respectful and culturally sensitive manner. Update patient records in real time to ensure that the care team has accurate, up-to-date information. Provide emotional support and reassurance to patients and families dealing with medical uncertainties. Attend training, workshops, or team meetings focused on professional development and enhancing cultural competency in patient care. Work collaboratively with multidisciplinary healthcare professionals to coordinate patient care effectively. End the day by reviewing unresolved patient concerns and ensuring follow-up as needed.
