The PCN Advanced Paramedic Practitioner role at Farnham Integrated Care Services (FICS), Farnham Centre for Health, requires a registered Paramedic with postgraduate qualifications, including the Independent and Supplementary Prescriber qualification and HCPC registration, acting within their professional boundaries.
This role involves working as part of a multidisciplinary team of AHPs, GPs, HCAs, etc., providing care for patients within our Urgent Same Day Care Service at FICS, supporting Farnham GP Practices.
The practitioner will handle initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care, demonstrating safe, clinical decision-making and expert patient care.
Main duties of the job
The PCN Advanced Paramedic Practitioner will attend to patients within a defined list of conditions, including assessment, diagnosis, and treatment, to deliver quality patient services alongside the daily duty doctor.
They will collaborate with the multidisciplinary team to meet patient needs, support service policy and procedures, and maintain training standards using appropriate equipment and facilities. The post holder will act as an autonomous allied healthcare professional within the PCN support framework at FICS.
Using advanced clinical skills, they will educate service users, promote self-care, and empower informed treatment choices. They will support integrated working to enhance efficiency and standardization.

Job responsibilities
* Provide a first point of contact for patients presenting with undifferentiated problems, utilizing history taking, physical examination, problem-solving, and clinical decision-making to establish diagnosis and management plans.
* Conduct assessment, treatment, and diagnosis at first contact within the Same Day Service at FICS.
* Make autonomous professional decisions, accountable for their actions.
* Refer patients to other care settings or treat and discharge as appropriate.
* Initiate diagnostic tests or investigations, interpret findings/reports.
* Generate EPS and FP10 prescriptions within scope, adhering to guidelines.
* Maintain professional standards per guidance from DH, HCPC, and COP.

* Implement service changes and pathways, collaborating with staff, PCN, and commissioners.
* Contribute to achieving quality targets and improving patient services.
* Participate in community health needs assessment and develop patient-centered strategies.
* Assist in developing new services and initiatives to improve existing ones.
* Support delivery of enhanced services and other PCN requirements.
* Participate in managing patient complaints and learning from incidents and near misses.
* Complete mandatory training and contribute to clinical governance.
* Attend annual appraisals with performance and training objectives reviewed regularly.
* Provide leadership fostering professionalism, compassion, and teamwork.
* Engage in training and service redesign activities to develop the role.
* Contribute to public health campaigns, such as flu clinics.

Person Specification
Qualifications
* Registered paramedic
* Evidence of ongoing professional development
Experience
* At least two years as an Advanced Paramedic
* Understanding of GPs and urgent care settings
* Knowledge of primary care prescribing and strategies for improvement
* Experience with telephone triage, managing minor injuries/illnesses, clinical examination skills, and managing chronic conditions
* Understanding of mentorship, clinical governance, and NHS landscape including PCNs and commissioners
