We are very excited to offer a Full-time, permanent Social Worker position within the Assertive Outreach Team in Stroud, based at Weavers Croft. The role involves working hours from 9-5, Monday to Friday, with occasional unsocial hours as needed.
We seek an experienced Social Worker familiar with care management and working with complex mental health patients. Weavers Croft is a community-based site collaborating with other teams to facilitate information, skills, and knowledge sharing.
The Assertive Outreach Team is a friendly, dynamic service providing secondary care to adults with complex needs who are reluctant to engage with mental health services. You will support clients’ families and significant others to achieve the best outcomes, particularly for those experiencing psychosis, substance or alcohol use, and at high risk of harm.
This service is currently under review, aligned with NHS England policies, and has recently received new recurrent funding, making this an exciting time to join an expanding team dedicated to high-quality, efficient, and effective care.
The role includes care coordination, working within CPA policies, adhering to risk procedures, travelling independently, utilizing IT skills, and understanding legislation related to safeguarding, MHA, and MCA.
Social Work Responsibilities
1. Arrange and purchase services to meet assessed needs, reassessing in line with Care Act requirements.
2. Undertake statutory, strengths-based assessments for personalized care planning.
3. Provide expertise on statutory and legal requirements, including the Care Act, MHA, Human Rights Act, and Housing Act, offering consultation to the team.
4. Contribute to service development within GHC Trust, including strengthening social care management and integration with the new Social Care Hub.
