Care Coordination is the team for you. We are offering an exciting position for a Band 6 Junior Sister to join the Care Coordination Team to work in our Hospital Inreach Service. The Care Coordination Inreach Team works across our acute and community sites to help streamline patient discharges and aims to achieve a safe, timely and seamless transition from hospital to home for our patients.
Responsibilities
* Visit the acute site daily (New Cross Hospital) attending a wide range of wards to identify patients who will need community support on discharge and take the lead with discharge planning for identified patients.
* Promote, deliver and ensure success of the inreach service with measurable set outcomes throughout the 12 months.
* Lead with discharge planning, including liaising with MDT, patients and their families to promote safe and timely discharges into the community.
* Promote service improvement and be motivated to deliver the inreach service with autonomy and leadership.
* Be ready to lead on changes to improve and develop the inreach service with support to develop skills and ideas.
* Work across community sites and travel as required to New Cross and West Park Hospitals.
Qualifications and Requirements
* UK driving license, business insurance and own a car for work purposes.
* Live NMC registration.
* Knowledge of acute and community services, including the Wolverhampton area, and a sound knowledge base of services delivered by a variety of organisations.
* Holistic triage and assessment skills to identify ongoing care needs for patients.
* Willingness to work autonomously and manage own working day.
