We are seeking a compassionate, motivated District Nurse to join our community nursing team in Guildford and Waverley.
This role is ideal for a registered district nurse who is passionate about delivering high‑quality, patient‑centred care in a community setting.
Responsibilities
1. Provide skilled nursing care to adult patients in their own homes, including those with long‑term conditions, complex care needs, and end‑of‑life care.
2. Conduct holistic nursing assessments and develop care plans using evidence‑based practice.
3. Deliver and evaluate nursing treatment, including venepuncture, wound care, catheterisation, syringe drivers, and pain management.
4. Use the EMIS electronic patient record system to maintain accurate, legible records.
5. Work autonomously and as part of multidisciplinary teams, coordinating with GPs, practice nurses, social care, local hospitals, specialist nurses, therapists, hospices, palliative care, mental health services, and voluntary sectors.
6. Attend multidisciplinary team meetings and Gold Standard Framework meetings.
7. Clinically manage a team of nurses and health care assistants, ensuring effective, efficient, safe and high‑quality care for a caseload of patients.
8. Plan, prioritise and make autonomous clinical decisions, including referrals to other agencies and admission/discharge from the service.
9. Perform physical examinations, including circulatory system assessment, Doppler assessment, venepuncture, catheterisation, rectal examination, and calculation for medication administration.
10. Manage medicines safely and legally, monitor regimes, assess effects, and advise patients on safe storage and disposal.
11. Triaging referrals and allocating work within the Community Coordination Centre, prioritising daily work and escalating issues as necessary.
12. Ensure equipment safety, infection control, maintenance and proper use of aids for patients and carers.
13. Communicate effectively with patients, families, carers, and teams, including out‑of‑hours and weekends, using mobile phone and email to provide timely responses.
14. Provide a positive, safe, high‑quality experience for patients, services users, families and carers.
Qualifications & Experience
Registered district nurse.
Experience in community nursing, especially with complex or end‑of‑life care.
Proven ability to work autonomously and within multidisciplinary teams.
Familiarity with EMIS or similar electronic health record systems.
Evidence of effective medication management and care planning.
Training & Development
We offer a structured preceptorship program and ongoing training and development pathways from Band 3 to Band 8.
