Social Worker within Hospital Team in Bromley, UK, earning £32 hourly. This role is a locum position for 3 months, starting from 24 February 2025. Full-time commitment is key. What you will do:
– Complete assessments under the Care Act 2014 and Mental Capacity Act 2005.
– Arrange care support systems and facilitate safe hospital discharges.
– Participate in daily board meetings to plan and discuss patient flow.
– Support younger adults with physical disabilities and older people, working collaboratively with carers.
– Work closely with health colleagues to ensure a holistic approach to patient care.
– Develop robust working relationships with partner agencies.
Candidates must be willing to work on site at the hospital, as remote working is not an option for this position. Your hands-on presence is vital for facilitating essential face-to-face interactions with vulnerable adults and multidisciplinary teams.
