We are looking for two Care Navigators to join our team at our Long Melford & Lavenham Practice sites, part of Unity Healthcare.
What’s the role?
1. You’ll be the first point of contact for patients accessing our surgery.
2. You’ll help ensure our patients can access care at the right time, with the most appropriate clinician.
3. The aim is to reduce reliance on GP time, reduce inappropriate referrals, and avoid unnecessary hospital admissions.
4. You’ll help enhance the care that our patients receive by handling queries related to appointments.
Unity Healthcare is run by Suffolk GP Federation CIC, a not-for-profit community interest company.

1. You’d be suited to this role if you have a caring and empathetic nature.
2. It’s important that you can work well as part of a team as well as independently.
3. You’ll need to be able to pick up new IT systems with supportive training.
Contract type –
Permanent Part Time. Both roles will work across both sites.
1. Role 1: 07:45 – 18:30 Wednesday and Friday – 19.75 hours per week.
2. Role 2: 07:45 – 18:30 Monday, Tuesday, and Friday – 29.25 hours per week.
