Are you a dynamic and compassionate pharmacy leader with a passion for improving the lives of older people and those living with frailty? An exciting opportunity has arisen for an experienced pharmacist working at/ towards consultant level to take on a strategic leadership role in this population across NHS Greater Glasgow and Clyde.
This pivotal role will provide clinical and strategic leadership in medicines optimisation for people living with frailty, with a particular focus on care home residents and those at risk of harm from inappropriate polypharmacy. You will play a key role in driving forward national and local priorities including My Health, My Care, My Home healthcare framework for adults living in care homes and will work collaboratively across health and social care interfaces to improve the quality, safety and experience of care for this population.
As the NHSGGC Lead Pharmacist for Frailty and Care Homes, you will:
1. Provide expert pharmaceutical leadership within the NHSGGC Care Home Collaborative.
2. Develop, implement and evaluate system-wide approaches and pathways to improve prescribing safety and reduce medication-related harm in frail populations.
3. Contribute to the development and delivery of person-centred deprescribing initiatives and polypharmacy reviews in line with Scottish Government priorities.
4. Influence and shape policy and practice at board level and beyond, working closely with HSCPs, GPs, care home teams, community pharmacy, and multidisciplinary partners.
5. Support innovation, workforce development and clinical education to build capability in medicines optimisation for older people.
About You:
We are seeking a GPhC-registered pharmacist with significant post-registration experience, including leadership roles in clinical pharmacy and demonstrable expertise in the care of frail populations including within care homes. You will be an excellent communicator, skilled in change management and collaboration across organisational boundaries. As the intention is to accredit this post as a Consultant Pharmacist you should already be credentialed in this field by the Royal Pharmaceutical Society or have made significant progress towards.
