Nurse Consultant Frailty Same Day Services

North Cumbria Integrated Care NHS Foundation Trust.

Location: 1 vacancy in Carlisle, 1 vacancy in Whitehaven.


About the role

We are looking for dynamic, motivated clinicians to join our newly funded Frailty Hub, a same‑day service for older people with frailty across Whitehaven and Carlisle. The Consultant Nurse or AHP will provide highly specialised clinical expertise, consultancy, and leadership to patients, carers, and Trust staff. The role requires close working with Emergency Department, SDEC, Acute Medical, and Virtual Ward teams, and includes out‑of‑hours support for weekend and evening decisions.


Main duties and responsibilities

* Lead as a senior decision maker within a composite workforce of frailty experts.
* Support and supervise a junior team in the assessment, diagnosis, and treatment of undifferentiated patients living with frailty.
* Spend time in the Frailty Hub, community hospitals, and in reach to Urgent Community Response.
* Maintain liaison with back‑of‑house teams to provide seamless transfer of patients when appropriate.
* Make senior clinical decisions for the Virtual Ward.
* Provide out‑of‑hours support for weekend and evening decisions.
* Provide highly specialised clinical expertise, consultancy, expert advice, and leadership to patients, carers, and Trust staff.
* Promote and improve care and services to older people across the Trust.


Qualifications

* Diploma/degree in a professional qualification for either an AHP or nursing profession.
* MSc in Advanced Clinical Practice and relevant subject area.
* Experience working at Advanced Practice level or above.
* Current registrant as required by HCPC/NMC.
* Non‑medical prescriber.
* Evidence of significant CPD, a well‑maintained CPD portfolio including recent postgraduate courses relevant to Frailty Same Day Care.
* PhD (preferred).
* Leadership training of a nationally recognised level suited for expert clinical leaders.


Knowledge and Experience

* Evidence of clinical work as an expert in this area and the ability to transfer highly specialist theoretical knowledge into expert clinical practice.
* Experience of effective multidisciplinary working at a senior level.
* Supplementary prescribing skills developed to enable appropriate prescribing in line with Department of Health guidelines.
* Highly advanced clinical reasoning skills.
* Lead role in Clinical Governance and its implications for services, including experience of quality issues and leading audit.
* Highly developed interpersonal skills with the proven ability to influence at all levels.
* Forward thinking, dynamic, self‑directed leadership qualities.
* Ability to influence others and inspire confidence in diverse professional backgrounds.
* Proven change management and quality improvement skills.
* Understand the legal responsibilities of the professions.
* Ability to organise and prioritise workload of self and others, motivating the multidisciplinary teams.
* Ability to cope while working in a stressful environment and with emotional or aggressive patients, carers, and staff.
* Proven research skills.
* Involvement in Equality Impact Assessment.
