Job Overview
We are looking for a dynamic, caring, enthusiastic person to work within our busy Intermediate Care Unit.
The role involves working as part of the Multi Disciplinary Team (MDT) comprising Nurses, Geriatrician Consultant, Ward Matron, Ward Manager, Lead OT, Physiotherapists, Pharmacist, and Healthcare Support Workers.
We are passionate about delivering excellent quality care to each patient and supporting their families and carers. Our goal is to work closely together to ensure safe discharges, helping patients regain their independence.
Main Duties of the Job
1. Maintain clear, concise communication with the MDT
2. Assist with washing and dressing patients
3. Support toileting and promote patient independence
4. Check patients' skin integrity and report abnormalities
5. Document care activities using System 1 (Computer System)
6. Provide detailed handovers using the SBARD approach
7. Maintain a clean, safe working environment
8. Contribute to the organisation's goals and values

Detailed Job Description and Main Responsibilities
Working closely with the team and reporting to the Ward Matron, Ward Manager, Ward Sisters/Charge Nurse. Supporting nurses, healthcare support workers, and therapy teams to deliver seamless patient care.
Responsibilities Include
* Washing, dressing, documentation
* Reviewing skin and wounds, escorting patients to appointments
* Clear communication, reviewing excretion patterns
* Supporting physical and mental wellbeing of patients and carers
Person Specification
Education and Qualifications
Essential criteria:
* Numeracy and literacy skills for accurate record-keeping and medication administration
* Spoken and written English to an appropriate standard
Desirable criteria:
* NVQ Level 3 or equivalent
Previous Experience
Essential criteria:
* Experience working with a multidisciplinary team
* Experience communicating with diverse populations
* Experience in a fast-paced environment
* Experience in a customer-focused setting
Desirable criteria:
* Healthcare/NHS or similar experience in community or hospital settings
* Caring roles, mental health settings, or working with adults with mental health issues
Skills and Knowledge
Essential criteria:
* Effective communication within the MDT, including handovers
* Maintaining accurate electronic and handwritten records
* Proficiency with Microsoft Word for drafting reports
* Understanding of the Code of Conduct for healthcare workers
* Contributing to care planning and assessment
* Working independently while following care plans and guidance
* Understanding consent, the Mental Capacity Act, and safeguarding legislation
* Supporting team objectives and delivering safe care
