This is a 12 month secondment role - therefore only open to qualified Advanced Clinical Practitioners.

Main duties of the job

The Advanced Clinical Practitioner will work within the Speciality of Medicine Division, independently seeing and treating patients with a variety of undifferentiated presentations within the assessment units and virtual ward.

You will be a key member of the ACP team working alongside both resident and senior doctors (including consultants) acting as an autonomous practitioner delivering a high quality service. Responsibilities will include regular assessments of patients using a range of assessment methods to determine ongoing plans of care whilst working collaboratively with a range of multi-agency and inter-professional resources.

You will also be responsible for independent reviews of inpatients who are receiving ongoing ward based care within the assessment units. This will be a developing role, with a staggered approach into the independent management of patients as the role holder’s competency, education, experience and confidence grows alongside a training, development and coaching plan.

You will be responsible for the supervision and training of trainee ACP’s. You will contribute to departmental governance and audit, and provide leadership and education to all levels of staff within the department.

Trainees will remain supernumerary for the duration of their training and will be provided with a clinical and educational supervisor.

Detailed job description and main responsibilities

You will be undertake daily assessments of patients using a range of assessment methods - history taking, holistic assessment, identifying risk factors, mental health assessments, requesting / interpreting diagnostic tests and conducting health needs assessments. Using expertise and decision making skills you will be responsible for initiating, evaluating and modifying a range of interventions which may include prescribing medications and therapies as well as lifestyle advice and care.

You will also be responsible for managing complex and unpredictable events supporting the team whilst ensuring the safety of patients, families and carers. You will act as a clinical role model / advocate for developing and delivering care that is responsive to changing requirements whilst maintaining effective professional and productive communication with patients, relatives, the multi-disciplinary team and external agencies demonstrating advanced communication skills.

Person specification

Knowledge & Experience

Essential criteria

1. Substantial acute/emergency medicine experience
2. Broad & advanced knowledge of clinical pathophysiology
3. Has the knowledge to support safe clinical decision making in relationship to clinical management plans
4. Proven leadership experience including delivering targets and service improvement
5. Sound level of knowledge in relation to invasive and non-invasive diagnostic and therapeutic procedures.
6. Leadership of teams or change initiatives.
7. Experience of clinical research

Desirable criteria

8. Experience in managing chronic health issues
9. Knowledge of more than one clinical area (medicine, surgery, general practice etc)

Qualifications & Education

Essential criteria

10. Professional clinical qualification
11. Current professional Body registration
12. MSc Advanced Practice / Evidence of level 7 study in relevant clinical field
13. Independent/Supplementary non-medical prescribing (or willing to work towards if trainee)
14. Evidence of continuing professional development
15. Advanced Life Support qualification (or willing to work towards if a trainee)

Desirable criteria

16. Teaching Qualification (7307/Certificate in Education/equivalent) or willing to undertake
17. Advanced Paediatric Life Support and Trauma Qualification (ETC, ATAAC, ATLS or equivalent)
18. Leadership/Management qualification or willingness to undertake.
19. Digital Badge/NHSE ACP Credentialing
