Primary Care Network Clinical Pharmacist (WN)

If so this is anexciting opportunity for you to join our existing pharmacy team, working with general practices across our PCN footprint,with support from our existing clinical teams to deliver excellent patient care. You will be partof our growing multi-disciplinary additional roles team, who all work togetherto support our GP practices and their patients.The rolescurrently include Care Coordinators, Clinical Pharmacists, PharmacyTechnicians, Paramedics, Health and Wellbeing Coaches, Social Prescribing LinkWorker, Outreach Nurses and Mental Health Practitioner.
Main duties of the job
The post holder will work within their clinical competencies as part of amulti-disciplinary team to provide expertise in clinical medicines management,provide face-to-face structured medication reviews, manage long termconditions, management of medicines on transfer of care and systems for saferprescribing, manage repeat prescription authorisations and reauthorisation,acute prescription request, while addressing both the public health and socialcare needs of patients in the GP practice(s) that make up the PCN.
The base for this role will be at Henry Moore Clinic, but working with all six practices within the PCN.

Person Specification
Qualifications
* Master's degree in pharmacy or equivalent
* Registration with the General Pharmaceutical Council (GPhC).
* Evidence of Continued Professional Development
* Independent Prescriber or willing to work towards
* Completion of the Primary care pharmacy education (CPPE) pathway or willing to work towards
* Postgraduate Diploma in Clinical Pharmacy (or equivalent)
Experience
* Evidence of working autonomously or as part of a team
* Experience of communicating with other healthcare professionals and the general public
* Experience of prioritising workload.
* Experience of using own initiative to solve problems
* Evidence of personal leadership to improve the quality of care
* Demonstrated ability to prioritise own workload and that of others
* Experience working in a primary care setting / PCN
* Understanding and knowledge of healthcare provision in GP practices, QOF, and PCN DES
* An appreciation of the new NHS landscape including the relationships between individual practices, PCNs and the commissioners
* Use of SystmOne
