Overall Purpose:

The post holder will work directly with Wyre Forest Housing Association Services’ clients who are experiencing mental ill health and wellbeing to support their needs relating to the wider determinants of health and wellbeing. They will act as a single point of access for Mental Health Services’ clients accessing voluntary sector and NHS mental health provision and signposting.

Clients will receive ONSIDE mental health support to that will improve their ability to sustain tenancies, avoid homelessness and secure accommodation as appropriate through person centred support that enables positive change. They will be encouraged to engage with: specialist services such as focussed Mental health support, any agreed personalised recovery plan, structured treatment for alcohol or drug use, neighbourhood and primary care mental health support, finance and benefit management and other community-based support as appropriate.

This role will be instrumental in enabling Mental Health Service clients to link up all elements of treatment and support and helping them to navigate both primary and secondary mental health services and wider community options.

This role will also involve supporting and providing information to Mental Health Services staff to enable better understanding of mental health pathways, thresholds, and sources of support, and also enable teams to adopt a supportive and empowering approach when working with people experiencing mental health challenges.

3>Key Responsibilities:
* To facilitate one point of access for support for Mental Health Services, VCSE and NHS support and signposting
* Triage clients and provide signposting or referral information
* Build and coordinate relationships and work closely with key staff in Mental Health Services, Neighbourhood Community Mental Health Teams, GP practices and Drug and Alcohol support service. Attend relevant meetings to encourage referrals and ensure that there is a clear understanding of the role of the Link Worker - Mental Health
* Help clients to identify and support them to address any wider issues which are impacting on their Mental Health status, health and wellbeing, such as debt, loneliness, diet, drug or alcohol use and make referrals to other services where appropriate
* Provide care navigation and facilitate access to community support, activities, organisations, opportunities and advice
* Assist clients to link up and coordinate all elements of treatment and support available including wider community support, information and advice
* Provide personalised one to one support to individuals using a range of evidence-based tools to support them to manage mental health and increase resilience
* Ensure the service is integrated within related provision, in particular Social Prescribing services, Individual Placement Support and recovery programmes
* Work as an integral part of the Mental Health Support team providing feedback as appropriate and contributing to the implementation of this new way of working
* Provide support to Mental Health staff to better understand mental health pathways, thresholds and sources of support. Encourage appropriate approaches and use of language when working with people experiencing mental health challenges
* Work with other VCSE delivery partners to ensure the provision of a seamless and integrated service ensuring recognition of anyone with an informal carer role
* Manage and prioritise caseload in accordance with needs, priorities and urgent support required by individuals
* Refer people back to other health professionals/agencies where appropriate
* Seek regular feedback about the quality and impact of the service, and prepare and provide regular written and verbal feedback and reports on performance
* Use appropriate systems for data capture, case management, feedback and reporting, to include NHS and Onside case management systems as appropriate, adhering to data protection legislation and data sharing agreements
* Work as part of the team to seek feedback, continually improve the service and contribute to business planning


Other Responsibilities:

* This role will involve working in different locations, and will require you to travel across Worcestershire, including isolated rural areas therefore the ability to drive and have access to a vehicle is essential.
* There may be occasional evening and weekend work which will be considered part of your normal working hours
