Responsibilities:
* Conducting comprehensive assessments to identify an individual's capabilities and needs.
* Developing support plans that prioritize what the service user wants to achieve (e.g., maintaining social links or living at home longer).
* Investigating concerns of harm or neglect and participating in Adult Support and Protection (ASP) activities.
* Collaborating closely with health colleagues in the Falkirk Health and Social Care Partnership and NHS Forth Valley.
* Managing a varied list of cases, particularly those involving long-term disability, elderly care, or complex mental health needs.
* Identifying risks to children and implementing protection plans where necessary.
* Working under the "Getting It Right For Every Child" framework to ensure holistic support.
* Building long-term, trusting relationships with families to facilitate positive change.
* Preparing detailed reports for legal proceedings and attending hearings to provide professional testimony.
* Participate in the "duty" system to handle incoming referrals and emergencies during office hours.
Requirements:
* PVG
* SSSC registration
* Social Work Degree

* Adults
