Main duties of the job
Provide excellent patient supportgreetingpatients, handling queries, and ensuring a positive experience at the surgery.

You will be joining a very supportive and well-establishedmultidisciplinary team consisting of4 GPs, 2 Advanced Nurse Practitioners(ANPs), a First Contact Physiotherapist (FCP), a Physician Associate (PA), 1Clinical Pharmacist (CP), a Practice Nurse, a Healthcare Assistant (HCA), andour experienced administrative staff.
Your Role Within the Team
The successful candidate will work closely alongside ourclinical and administrative teams to help deliver high quality, compassionatecare to patients accessing primary care services. You will play an importantpart in maintaining smooth day to day operations, supporting both patients andclinicians, and contributing to the warm, collaborative atmosphere that makesPhoenix Family Care a great place to work.
Job responsibilities
The following are the core responsibilities of thePractice Nurse. There may be, on occasion, a requirement to carry out othertasks. This will be dependent upon factors such as workload and staffinglevels:
* a. To support the Infection Prevention Control(IPC) lead and to work to the IPC Policy supporting the implementation ofaudits, training, support and evidence of compliance to the practice manager
* b. To support the production of the IPC annualstatement in conjunction with the practice manager
* c. To develop, implement and embed an effectivecare management programme for the frail and housebound patients registered atthe practice
* d. To assess the needs of patients ensuring theprovision of healthcare is appropriate, incorporating evidence-based practice
* e. To develop, implement and embed healthpromotion and wellbeing programmes
* f. To implement and evaluate individual treatmentplans for chronic disease patients that promote health and wellbeing
* g. To identify, manage and support patients atrisk of developing long-term conditions, preventing adverse effects topatients health
* h. As required, to provide routine nursing careto patients in accordance with clinical based evidence, NICE and the NSF
* i. As required, to diagnose and manage acute andchronic conditions
* j. To prescribe medication for patients inaccordance with PGDs and own scope of practice
* k. To review patient medications to enhancecompliance
* l. To provide guidance to patients in the use ofprescribed and over-the-counter medications regarding side effects anddosages
* m. To co-ordinate the provision of travelmedicine services
* n. To request pathology services and processpathology results as required
* o. To oversee the provision of chronic diseaseclinics, delivering patient care as necessary, referring patients tosecondary/specialist care as required
* p. To maintain accurate clinical records inconjunction with extant legislation

* aa. To support patients in the use of theirprescribed medicines or over-the-counter medicines (within own scope ofpractice)
* bb. To liaise with external services/agencies toensure the patient is supported appropriately (vulnerable patients etc.)
* cc. To delegate clinical responsibilitiesappropriately (ensuring safe practice and the task is within the scope ofpractice of the individual)
* dd. To support the clinical team with allsafeguarding matters, in accordance with local and national policies
* ee. To understand practice and local policies forsubstance abuse and addictive behaviour, referring patients appropriately
* ff. To deliver opportunistic health promotionwhere appropriate
* gg. Regularly monitor and check medical equipmentand report faults to the practice manager.
