Role Overview
An exciting opportunity has arisen for 2 experienced Band 7 Social Workers to join a busy NHS service in Torbay.
You will be working within a multidisciplinary team, supporting complex adult cases and playing a key role in assessment, safeguarding, discharge planning and care coordination.
Key Responsibilities
Complete Section 9 Care Act assessments and develop support plans
Undertake Mental Capacity Assessments (MCA)
Lead Section 42 safeguarding enquiries
Carry out comprehensive risk assessments
Complete Carers assessments
Support hospital discharge planning, including funding pathways and commissioned services
Manage complex caseloads and contribute to MDT decision-making
Provide guidance and supervision where required
Maintain accurate and timely documentation
Requirements
Qualified Social Worker (registered with Social Work England)
Strong knowledge of:
Care Act 2014
Mental Capacity Act 2005
Experience working within adult social care / hospital discharge settings
Ability to manage complex cases and work autonomously
Experience in safeguarding and risk management
Strong communication and report writing skills
Confident using IT-based case management systems
Key Skills
Ability to lead on complex cases and decision-making
Strong MDT working and stakeholder engagement
Excellent organisational and time management skills
Ability to work under pressure and meet deadlines
Commitment to person-centred and anti-discriminatory practice
