Under the direction of the Lead Midwife for Patient Safety the post holder will be responsible for the provision of practical support, expertise and facilitation to the Service Line, across a wide range of patient safety and quality improvement activities. The post holder will ensure that the Service Line is appropriately supported in order to deliver their Quality Governance agenda in line with the Patient Safety Incident Response Framework (PSIRF).
Responsibilities:
* Provide support relating to the management and investigation of Patient Safety Incident Investigations (PSIIs), incident reporting, risk registers, clinical audit plans and clinical effectiveness activity.
* Work closely with the Quality Governance teams (patient safety, audit & effectiveness etc.) and act as a link between the Trust Management Directorate and the Service Line.
* Support governance meetings and assist the Lead Midwife for Patient Safety and other key personnel in implementing the Quality Governance Framework, dashboards and reports; report to the Lead Midwife for Patient Safety with oversight from senior colleagues as required.
* Assist in maintenance of the Service Line risk registers in liaison with senior staff and Patient Safety team.
* Ensure incident reports involving or affecting Women\'s Services are actioned in accordance with PSIRF; identify and communicate high scoring risk issues and incident trends; produce monthly incident reporting reports to support specialty review of trends and shared lessons learned.
* Ensure an appropriate action planning process for incidents and near misses, with monitoring and dissemination of lessons learned.
* Be conversant with SEIPS and human factors; lead or advise on the most appropriate method/process of investigation.
* Support 2nd victims (staff involved in an incident) through engagement, de-briefs and sign-posting to Trust resources; foster a just culture.
* Support specialty activities related to improving compliance with CQC and MIS Standards and other assessments; provide information for compliance assessments.
* Monitor key performance indicators showing improvements in risk management; provide feedback on incident reporting to staff; assist in developing and monitoring action plan time scales and provide assurances to governance groups.
* Ensure evidence and documents are up-to-date and compliant with Trust policy; raise the profile of quality governance and patient safety within the Service Line; promote a learning culture and psychological safety.
* Promote open, honest and trusting culture to encourage incident reporting and multidisciplinary learning; embed and implement action plans following PSIIs.
* Work with the Specialty management team to plan governance meetings and ensure relevant topics/papers are covered; ensure arrangements to implement Trust governance procedures such as investigations, complaints and risk assessments.
* Coordinate complaints investigations and ensure actions and lessons learned are identified and implemented; attend complaints meetings as necessary and liaise with the complaints team within Divisions.
* Develop and maintain a clear programme of work for the Service Line\'s Quality Governance agenda, including dashboards and programmes of work; assist in preparing Specialty reports for Quality Governance panels and other meetings; contribute to other Quality Governance reporting as appropriate.
* Ensure work programmes align with Trust and national priorities (CQC registration, MIS, and other external assessments); collaborate on evidence collation for external reviews or assessments; support development and implementation of quality governance reporting structures and meetings.
* Meet regularly with senior Quality Assurance and Governance leads to align priorities and standards.
* Join the Bedfordshire Hospitals NHS Foundation Trust, with two site operation and approximately 7,500 staff serving around 700,000 people; travel between hospital sites may be required.
* All new staff will be subject to a probationary period covering the first six months in post.
Qualifications/Experience (as described in the role):
* Experience in patient safety and quality improvement activities; ability to support governance functions and incident management.
* Knowledge of PSIRF, clinical governance, risk management, and quality systems.
* Ability to prepare and present dashboards, reports and action plans; strong communication and collaboration skills to work with multidisciplinary teams.
* Understanding of SEIPS/human factors and willingness to lead or advise on investigations.
* Ability to work across hospital sites and manage competing priorities; willingness to travel as required.
