Responsibilities
* Support the Discharge to Assess process, ensuring safe and timely discharges.
* Help maintain flow and capacity across the Trust.
* Work across all clinical areas and provide cover at King’s Mill Hospital, Mansfield Community Hospital, and Newark Hospital.
Qualifications
* Excellent communication skills and a calm, adaptable approach.
* Self‑motivated and able to respond quickly to changing situations.
* A clear understanding of discharge pathways and awareness of social care and health processes (full training provided).
* A full driving licence is essential as you’ll travel between sites.

Working Pattern
Seven‑day service, including bank holidays. Shifts include 8:00 am–4:00 pm and 8:30 am–4:30 pm.
