Job Description:
Your responsibilities include:
* Providing accessible information and advice through an immediate response service.
* Ensuring Fair Access to Care guidance is applied for all citizens as per Monmouthshire County Council.
* Working with individuals to identify risks to their independence using various tools.
* Exploring options to support independent living in communities.
* Developing solutions and personal support plans with individuals.
* Providing information about funding options to facilitate informed choices.
* Carrying out financial assessments following local and national frameworks.
* Maintaining accurate financial records and managing delegated budgets.
* Collaborating with communities, providers, and partner organizations to develop community-based solutions.
* Partnering with other agencies to ensure quality service delivery.
* Serving as a case coordinator when appropriate.
* Delegating and supervising tasks by support workers.
* Documenting casework accurately and regularly according to departmental procedures.
* Providing specialist social work assessments, including Continuing Health Care Assessments.

* 2.3 Knowledge of informal carers' needs.
* 2.4 Experience working with diverse needs.
