Typical day as a Support Coordinator
Creating support plans with your customers and coordinating our small team of Support Workers working to the plans.
Customer group - Single homeless men with low to medium support needs
Supporting customers with a variety of needs such as, budgeting, maintaining their tenancy, substance misuse, offending and moving onto independent accommodation.
Carrying out risk assessments, support planning, goal setting and regular reviews.
Cuppas, chats and catch ups. Theres no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

You bring
Passion to support our customers to live their best life, working collaboratively with an eye for detail.
Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.
Experience of coordinating and assessing customer referrals.
The ability to workon your own initiative, remaincalm under pressure and havea resilient approach
Job details
Where were flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
Work pattern will be on a rota basis including some weekends
Although youll lone work at times, we do get together regularly [in the daytime] for training and team meetings.
Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
Youll need an Enhanced DBS check done and we pay for that.
