Job overview
This role is within the Conwy Community Mental Health Team for Older Persons, focusing on clients aged 70+ with organic and functional illnesses such as dementia, anxiety, and depression. The nurse will assess, implement, and evaluate care plans, and manage risks, working closely with psychiatrists, social services, inpatient wards, and third-sector agencies.
Main duties
Assess and manage care for older adults with mental health issues, coordinate with multidisciplinary teams, and ensure appropriate care environments.

Qualifications
* NMC Registered Nurse Part 3 or Part 13
* Diploma or Degree in relevant subject
Experience
* Significant post-registration experience at Band 5 or above
* Relevant clinical experience
Knowledge
* Up-to-date knowledge of the Mental Health Act and Mental Health (Wales) Measure
* Evidence of ongoing professional development
Skills
* Ability to work autonomously
* Teamwork skills
* Leadership and people management skills
