Doncaster North PCN are seeking an experienced LeadClinical Pharmacist to develop our Primary Care Network pharmacy team. The postholder will work across multiple GP practices in our network deliveringadvanced clinical medicines optimisation, structured medication reviews, andmedicines safety leadership. This is a senior role requiring autonomouspractice as an independent prescriber, with responsibility for complexpatients, service development, quality improvement and leadership within themultidisciplinary team.
Main duties of the job
Deliverface-to-face and remote structured medication reviews, focusing on polypharmacymultimorbidity care home residents and vulnerable patients
Actas an independent prescriber, implementing prescribing changes and orderingappropriate monitoring
Leadrepeat prescribing systems, reauthorisations and medicines optimisation ontransfer of care
Managemedicines reconciliation following hospital discharge and reducemedicines-related admissions
Leadon medicines safety and governance
Driveand deliver quality improvement audit QOF and CQC medicines-related activity
Supportpopulation health initiatives and cost-effective prescribing programmes
Provideexpert medicines advice and education to GPs MDT colleagues and patients
Supportthe wider pharmacy team
Actas the designated pharmacy supervisor for trainee pharmacy students
Workcollaboratively with PCN leadership ICB pharmacists and secondary care teams

Job responsibilities
The following is not an exhaustive list and the post holder may at times need to undertake duties as per the needs of the PCN
Lead a team of Clinical Pharmacists team and demonstrate the characteristics of a role model to members in the team providing support to team members and ensure there is appropriate clinical supervision in place for all pharmacists
Undertake clinical supervision and systematic peer caseload review of colleagues on an individual or group basis
Ensure all pharmacists are engaged with the review and appraisal systems within the PCN
Act as a role model demonstrating effective clinical leadership on a daily basis and promote the behaviours of being proactive positive respectful supportive reliable and trustworthy
Conduct teaching and assessment effectively according to a learning plan with supervision from a more experienced colleague
Understand the implications of national priorities for the team and/or service and manage the team of Clinical Pharmacists through these changes
Assess and monitor risk in own and others practice acting on results thereby ensuring safe delivery of care
Follow professional and organisational policies and procedures relating to performance management
Participate in maintaining systems for risk assessment and minimisation including promoting a fair blame culture in order that near misses and incidents are reported and investigated
Contribute to investigation of incidents and complaints when required; participate in identifying lessons learnt and the sharing of learning across the organisation
Ensure dignity privacy and cultural and religious beliefs are respected at all times
Continually monitor standards of care through quality markers achievement of PCN targets and benchmarking
Be aware of local quality data and use it to identify practice development required
Work proactively in managing change in own speciality to improve practice and health outcomes
Identify and provide leadership on areas of prescribing and medicines optimisation Conduct clinical audits and improve projects or work with colleagues such as PCN staff Practice staff etc
Present results and provide leadership on suggested changes
Contribute to national and local research initiatives
Clinical Responsibilities
Undertake structured clinical medication reviews for patients with polypharmacy multiple comorbidities or specific clinical needs
Carry out medication reviews in care homes working closely with staff to improve safety in medicines ordering and administration
Perform domiciliary medication reviews attending and referring patients to multidisciplinary case conferences when appropriate
Manage a caseload of patients presenting with minor self-limiting conditions within clinical competence referring where needed
Run face-to-face and telephone clinics to support patients with medicine-related concerns or queries
Prescribing and Medicines Management
Act on acute and repeat prescription requests ensuring appropriate authorisation monitoring and safety
Reconcile medications following discharge from hospitals care homes or intermediate care identifying discrepancies and ensuring continuity
Analyse use of high-risk medicines linked to unplanned admissions and implement prescribing changes to reduce risk
Monitor and action prescribing alerts from the MHRA local medicines optimisation teams or formulary updates
Implement and audit practice compliance with local/national prescribing guidelines and NICE technology appraisals
Medicines Optimisation and Safety
* Identify patients at risk of medicine-related harm using clinical systems and data analysis tools
* Carry out clinical audits and quality improvement projects in priority prescribing areas
* Provide advice and input into practice-level formulary decisions and maintain formulary compliance
* Develop and implement repeat prescribing policies including managing reauthorisations and monitoring test requirements
Communication and Patient Support
Respond to complex medicine-related queries from GPs practice staff community pharmacists and patients
Provide follow-up to patients after medication changes to monitor response and manage concerns
Support patients post-discharge to understand and use their medicines effectively
Leadership and Service Development
Provide clinical leadership in medicines optimisation across the practice/PCN
Contribute to the development and implementation of services involving medicines e g treatment pathways
Analyse and present prescribing data to inform decision-making and improve safety
Participate in local medicines management groups or committees as needed
Care Quality Commission CQC and Governance
Support practices in meeting CQC requirements where medicines and prescribing are involved
Ensure adherence to best practice and governance standards related to medicines use and storage
Signposting and Integration
Ensure patients are referred to the appropriate healthcare professional or service in a timely and effective manner
Foster strong links between general practice community pharmacy and hospital pharmacy teams to ensure cohesive care and efficient skill mix utilisation

Person Specification
Experience
* Strong leadership, communication and analytical skills
* Enthusiasm to join and develop a fantastic team in a dynamic forward thinking PCN
* Experience in medicines optimisation, audit, governance and service development
* Minimum 2 years post-qualification experience, including advanced practice in primary care
* Experience with Clinical Systems such as EMIS & Systmone
Qualifications
* Registered with the General Pharmaceutical Council GPhC
* MPharm or equivalent with postgraduate clinical qualification
* Minimum 2 years post-qualification experience including advanced practice in primary care
