Offer description

About the Role

As a Social Care Assistant in Connect to Community, you will provide practical, frontline support to adults and carers at the first point of contact with Adult Social Care. You will gather relevant information and offer advice, guidance and appropriate signposting, while supporting strengths‑based conversations that promote independence and wellbeing. You will assist with, or carry out, proportionate assessments with the right level of support and contribute to duty and intake activity, including responding to urgent enquiries. You will play a key role in signposting residents to community, voluntary and universal services, supporting access to reablement, technology‑enabled care and equipment, and identifying any safeguarding concerns so they can be escalated appropriately. The role also involves working collaboratively with colleagues and partner agencies to ensure coordinated and effective support. Alongside this, you will manage a varied workload, maintain accurate and timely records, and consistently demonstrate a professional, person‑centred approach in your work.
