We have an exciting opportunity for experienced Registered Nurses to join our new Integrated Discharge Team.
The Discharge Coordinator role is crucial to managing and facilitating patients' safe and timely transitions from hospital to their next destination, such as home or another facility. Key responsibilities include coordinating multidisciplinary teams (doctors, nurses, social care, third sector partners), identifying and resolving potential barriers to discharge, ensuring patients have necessary equipment and support, maintaining accurate records, and liaising with families and external services.
We are seeking motivated and adaptable Registered Nurses who would like to be part of a friendly and supportive team. The post holders will be confident decision makers, be able to work without direct supervision, and be able to work across Multi-disciplinary Teams within Health & Social Care.
The role requires you to have full UK NMC registration and be flexible to work weekdays and weekends on a rotational basis to meet the needs of the service.
