Advanced Nurse Practitioner (ANP) – County Durham & Darlington NHS Foundation Trust. Closing date: 06 November 2025.
We are looking to expand our team and have an exciting opportunity for a dynamic, highly motivated, innovative and forward-thinking Advanced Nurse Practitioner (ANP) to provide clinical expertise in the assessment, development, planning, implementation and monitoring of patient care with the community hospitals across County Durham.
The post holder will require specialist knowledge to demonstrate safe, competent clinical decision making and expert patient care including diagnostic skills. Be a lead specialist in a defined area of nursing care. Provides education and training to other staff, students and/or patients/clients. Undertakes research and leads clinical audits in own specialist area.
Responsibilities
 * Work alongside Medical Practitioners and the Advanced Clinical Practitioner to use clinical knowledge and experience with sound clinical reasoning to interpret information from patients, relatives/carers or referring sources to make clinical decisions, provide differential diagnoses and create records to support consultation periods.
 * Collaborate with senior decision-makers to agree the most appropriate outcome for the patient and expedite care.
 * Be a clinical role model, contribute to the development of the team and your own professional growth.
 * Demonstrate strong leadership, effective problem solving, and excellent communication with all stakeholders; maintain a proactive and positive approach.

We provide hospital services from two acute sites — Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community-based settings and care in patients’ homes.

Person Specification
Experience
 * Knowledge, clinical skills and experience in older persons management
 * Frailty/older persons care
Special Requirements
 * Ability to work hours flexibly when service needs require
 * Ability to travel independently across sites to fulfil the requirements of the post
Special Skills & Knowledge
 * Experience working at Nurse Advanced Level or demonstrates ability to progress to this level
 * Has acquired the experience and training to support the above requirements
Statutory Registration
 * Registered Nurse (adult or Child) on part 1 of the NMC register
 * Valid UK NMC PIN
Qualifications
 * Evidence of professional development
 * Educated to Degree level or working towards
 * Teaching and mentoring qualification
 * Clinical skills course
 * Non-medical prescribing or a willingness to undertake
