Responsibilities
* Deliver day‑to‑day evidence‑based nursing care and case coordination for children and young people aged 0‑18 with acute, short‑and long‑term nursing needs.
* Adopt a person‑centred care approach, encouraging active engagement to empower families and promote independence.
* Work within an integrated community setting to prevent hospital admissions, support early discharge, and reduce readmissions.
* Collaborate with parents to produce a joint plan of care that meets the child’s needs and empowers them to look after their child with our support.
* Be a member of the supportive, friendly and fun specialist Team, contributing to the trust’s culture of excellence and inclusion.
Qualifications and Experience
* Excellent communication, interpersonal and organisational skills.
* Motivated individual with a commitment to high‑quality, patient‑centred care.
* Registered nursing qualification and relevant paediatric community experience (as required).
