The Care Coordinator responsibilities include but are not limited to the following: Raise awareness of how to identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared to shared decision-making conversations. Administrative management of vaccination invites and housebound patients. To assist the Practice Manager and Business Manager to manage other clinical, LES,DES programmes. Attend and participate in any practice meetings when required To undertake any other activities that may from time to time be reasonably requested by the Business Manager, the Nurse Manager, the Practice Manager or the Partners.
