This post is fixed term/secondment until 31/03/2027 due to the needs of the service. If you are a Hywel Dda University Health Board employee and are applying for this post as a secondment, agreement for the secondment is needed from your current manager prior to application.
There is a 1 x full time post (37.5 hours per week) covering South and Mid Pembrokeshire. There is 1 x part time post (18.75 hours per week) covering North Pembrokeshire.
There is an opportunity for development within this post to support the progression on to apply for the Specialist Community Public Health Nursing Qualification. The post holder will work as part of a skill mix team with the Health Visiting Team and will support the delivery of the Healthy Child Wales Programme from 0‑5 years old.
Responsibilities
 * Support the Healthy Child Wales Programme.
 * Deliver health promotion advice, facilitate healthy baby clinics, complete delegated developmental assessments, and participate in training.
 * Work collaboratively with other disciplines and agencies.
 * Deliver components of the Health Visiting Core programmes to families with pre‑school children.
 * Assess health needs and develop, implement and evaluate programmes of care for children and families.
Qualifications & Experience
 * Registered Children Nurse Qualification or an Adult Registered Nurse Qualification.
 * Ability to travel between sites in a timely manner.
 * Ability to speak Welsh is desirable. English and/or Welsh speakers are welcome.
 * Professional working relationships with other statutory and voluntary practitioners.
