Overview

The post holder will work as part of a friendly, multi-disciplinary team in a patient-facing role and will take responsibility for areas of chronic disease management. They will undertake structured medication reviews to proactively manage patients with complex polypharmacy and those with multiple co-morbidities. They will support general practice staff with prescription and medication queries, undertake medicines reconciliation on transfer of care, and help implement systems for safer prescribing. The role includes medicines optimisation and involvement in quality improvement programmes. The post holder should understand current educational policies relevant to working areas of practice and keep up to date with clinical practice. Strong relationships with practice and PCN staff are essential to improve quality of care and operational efficiency.


Main duties

* Undertake medication reviews and work within your scope of practice to implement changes or produce recommendations for/refer to other clinicians to implement if outside scope.
* Undertake clinical audits of prescribing identified internally by the Advanced Practitioner Clinical Pharmacist and practice, and externally by quality scheme contractual requirements; feedback results and implement changes with the practice team.
* Implement changes to medicines resulting from MHRA alerts, product withdrawals, and other local/national guidance.
* Manage a caseload of patients within scope of practice and limits of competence.
* Signpost to community pharmacy and other healthcare professionals.
* Provide patient-facing and telephone clinics to answer questions about medicines.
* Reconcile medicines after hospital discharge or admission, rectify changes, and work to ensure patients receive the medicines they need.
* Set up and manage systems to ensure continuity of medicine supply to high-risk patients.
* Answer enquiries, propose solutions, and provide follow-up to monitor effects of changes.
* Ensure systems are in place for drug monitoring and streamline where possible.
* Advise on development and implementation of new services with medicinal components.


About the employer

Congleton and Holmes Chapel (CHOC) Primary Care Network (PCN) serves a population of around 49,500 across Congleton and Holmes Chapel. The PCN includes clinical pharmacists, SPLWs, ANPs, paramedics, first contact physiotherapists, and mental health practitioners, with ongoing research and a PCN Education Clinical Lead providing monthly mentorship. The team is dynamic and aims to deliver high-quality local services across health, social care, and the voluntary sector.

Role: PCN Clinical Pharmacy (part-time, 18.75 hours per week)

Location: Multiple sites in Congleton and Holmes Chapel, Cheshire

Salary: £46,500 per year FTE (Starting from £46,500) Contract: Fixed term; Duration: 1 year

Person specification

* Qualifications
Essential: General Pharmaceutical Council registration; undergraduate degree in pharmacy; commitment to CPD.
* Desirable: Postgraduate Diploma in Clinical Pharmacy; independent prescribing qualification.
* Experience
Essential: Qualified pharmacist with practice portfolio; experience with common acute and long-term conditions in general practice; ability to collaborate with GP, community, and hospital pharmacy; experience of partnership working; understanding of prescribing audits.
* Skills and capabilities
Ability to communicate complex information, knowledge of evidence-based healthcare, understanding of health determinants, knowledge of IT systems, and ability to plan, manage, monitor, and review medicines optimisation for long-term conditions; ability to analyse complex information; risk management and information governance awareness; confidentiality and data protection.
* Behaviours
Inspirational, flexible and proactive; able to work autonomously and in a team; professional, patient-focused; remains calm under pressure; adaptable to shifts; commitment to high patient care standards.
* Communication skills
Ability to use diverse communication methods; build relationships at all levels; excellent interpersonal skills; respectful handling of conflicts.
