Overview
Once a person has been discharged from hospital the team have the responsibility for completing Care Act Assessments for individuals to ascertain an individual’s ongoing needs for care and support. As a social worker in hospital discharge you will play a pivotal role in ensuring that the individuals we work with receive the support they need to thrive.
This position offers an exciting opportunity to make a meaningful impact in the lives of others and contribute to the success of discharge from hospital.
A sound understanding of the Mental Capacity Act is essential and it would be beneficial for individuals to have experience of completing complex mental capacity assessments.
This post is being offered as either a Fixed Term or Secondment opportunity until 30th November 2026.
