The successful candidate will play a key role in supporting the multidisciplinary team and helping to shape the future of primary care services. Key Responsibilities Manage long-term conditions such as diabetes, asthma, and COPD through routine reviews and proactive care. Deliver cervical screening, contraceptive advice, and sexual health services. Administer childhood and travel vaccinations, as well as seasonal flu and adult immunisations. Provide wound care, treatment room services, and request relevant pathology tests. Promote preventive health education, self-management, and lifestyle awareness. Contribute to the Quality and Outcomes Framework (QOF) and local incentive schemes. Collaborate effectively with GPs, HCAs, Nursing Associates, and Pharmacists to deliver coordinated care. Uphold high standards of infection control, clinical documentation, and patient safety.About You We'd love to hear from you if you: Are a Registered General Nurse (RGN) with current NMC registration. Have recent experience working in a GP practice or primary care setting. Are confident in chronic disease management, immunisations, cytology, and wound care. Have excellent communication skills and thrive in a multidisciplinary team. Take a proactive, patient-centred approach to your work.
