Main duties of the job
The post holder will collaborate with the multidisciplinary team to implement and evaluate a seamless patient transfer of care service, ensuring high standards of care. Responsibilities include assessing, monitoring, and reporting on patient length of stay, delayed transfers of care, and related practice issues. The post also involves case managing the discharge processes for patients referred via the Integrated Falls and Fracture Prevention Service (IFH), ensuring referrals are complete and accurate, and directing them to the appropriate Hub or Single Point of Access (SPA) for Pathways 1, 2, and 3 discharges.
