About the role
Our Foundation Pharmacists work on a rotational basis and receive training and supervision through collaborative initiatives between Torbay and South Devon NHS Foundation Trust (TSDFT) and Devon Partnership NHS Trust. The rotation involves medicine optimisation services across acute, community, intermediate, and mental health settings, with the aim of providing clinical pharmacist input to patients across all sectors of the healthcare system.
The Foundation Programme delivers a structured pathway to develop clinical and professional skills, supported by senior clinical pharmacy teams. Rotations are designed to build competence to practise safely and autonomously at a foundation level, providing patient‑centred, evidence‑based, and cost‑effective pharmaceutical care.
Responsibilities
* Develop and maintain effective working relationships that inspire confidence in clinical pharmacy and the department.
* Actively participate in the Foundation Programme to achieve the defined skills and competencies.
* Collaborate with pharmacy technicians and assistants to provide support for discharge facilitation and reuse of patients’ own medicines.
* Lead and support quality‑improvement projects and audits related to medicine optimisation and prescribing.
* Provide timely and accurate advice on prescribing issues and evidence‑based prescribing in accordance with local and national guidance, including the South and West Devon Joint Formulary.
Qualifications & Requirements
* Registered or trainee pharmacist eligible to register by 2026.
* Commitment to continuing professional development and participation in the Foundation Programme.
* Capability to practise safely and autonomously at a foundation level across multiple clinical settings.
* Demonstrated flexibility, positivity, and problem‑solving ability.
