Discharge and Flow Team - Empowering Independence
Reablement is a short-term, therapist-led service focused on promoting independence and reducing ongoing service needs. We provide information, needs analysis, equipment, adaptations, and targeted support.
Since March 2020, we have collaborated with health services to improve hospital discharges in Wiltshire. The Wiltshire Locality Hub manages discharges seven days a week from hospitals and other facilities.
This role offers variety and flexibility within a supportive team environment.
As a Locality Coordinator, you will coordinate hospital discharges at the Wiltshire Locality Hub, working with NHS partners and hospitals to ensure smooth processes. The role includes weekend work as part of the service operation.
Applicants with a background in health or social care, especially those experienced in supporting adults at home, are encouraged to apply. Knowledge of adult care legislation, safeguarding, and social care transformation is essential.

Job responsibilities
The Locality Coordinator will work according to the Workforce Quality Assurance Framework, supporting efficient service delivery in line with policies, standards, and procedures. You will triage incoming work, coordinate discharges, and liaise with health and social care partners to facilitate safe hospital discharges for a population of approximately 160,000 residents, with an active customer base of 1,000-1,500.
Specific duties include:
* Review customer situations, assess risks, and coordinate discharge plans.
* Identify eligible needs and outcomes, including prevention services.
* Collaborate with customers and agencies to develop support and discharge plans.
* Coordinate discharge processes, acting as a link between hospitals, providers, and community services.
* Manage a caseload, working with occupational therapists and prioritizing tasks effectively.
* Provide administrative support, including data collection and reporting.
* Apply a person-centered approach, respecting individual circumstances and environments.
* Update progress regularly and adapt plans as needed, consulting occupational therapists for complex issues.
* Arrange equipment via Medequip to support discharges and promote independence.
* Utilize community resources to minimize long-term funded services.
* Consider family contexts, referring to additional support organizations as appropriate.
* Advise customers on financial eligibility, referring to finance and benefits teams.
* Accurately record contacts and discharge information, maintaining confidentiality and data protection standards.
* Ensure proper use of systems such as SystemOne and Adult Services excel.
* Facilitate collaboration with health teams, hospitals, and GP surgeries.
* Maintain effective communication with all stakeholders.
* Follow safeguarding policies, making referrals and participating in investigations.
* Engage in supervision, training, and ongoing professional development.
* Adhere to complaints procedures and represent the Council positively in all interactions.
