An exciting opportunity has arisen for a part time Band 6 Physiotherapist to join Whitby Community Therapy Team. Our multidisciplinary team provides therapy and rehabilitation to the Whitby Hospital inpatient unit and in the community in the surrounding towns, villages, and rural communities of the Yorkshire Coast. This is a community physiotherapist role with an exciting opportunity to be involved in implementing an intermediate care model in Whitby. We are willing to offer a relocation package to suitable candidates if you do not live locally and are interested in the available position. We can discuss relocation expenses and incentives upon appointment. Responsibilities
 * As a senior therapist, you will have management and supervision responsibilities for junior staff and students, and contribute to service development.
 * Triage and prioritise new referrals received by the service.
 * Be responsible for a designated caseload of patients: organise this effectively and efficiently with regards to clinical priorities and time management.
 * Undertake comprehensive assessments of patients, including those with diverse or complex presentations, using clinical reasoning skills.
 * Support patients with individualised goal setting, education and care plans to work towards fulfilling their potential, via delivery of evidence based clinical interventions and practice in partnership with patients, relatives, staff, managers, and externally with other agencies.
 * Be accountable in your area of clinical responsibility, participating in regular clinical and line management supervision, completing required training and ensuring ongoing professional development.
 * Have a Home First ethos and provide outreach into the community for patients recently discharged from rehabilitation inpatient unit.
 * Support innovation and service development.
 * Rotation of intermediate care and UCR, plus any ad hoc cover required on inpatient ward.
