Clifton Medical Centre are looking to recruit an enthusiastic and dedicated Practice Nurse who specialises in Diabetes to compliment our friendly and dynamic team.

Located in the centre of Rotherham and with a branch surgery at Wickersley, we serve a varied population of approximately 13,400 patients.

This is an excellent opportunity to make a meaningful impact on the lives of patients with diabetes by providing specialist care, structured reviews, and expert guidance to help them manage their condition effectively.

As a key member of the team you will assess, treat and support patients, optimising treatment plans and empowering them with the knowledge to take control of their health. You will also contribute to service development, ensuring high standards of diabetes care within the practice.


Main duties of the job

The successful candidate would work to ensure a high level of care is delivered to our patients with diabetes. The practice is very supportive of any development opportunities.

A key aspect of the role is patient education, supporting individuals in self-managing their condition and promoting adherence to treatment plans. You will establish and maintain effective communication with patients, carers and the wider healthcare team, acting as a resource for specialist diabetes advice.

You will be expected to maintain accurate patient records, participate in clinical audits and quality improvement initiatives, and contribute to the development and implementation of practice protocols and guidelines to enhance diabetes care and overall patient management.

Clifton Medical Centre is a well-established and dynamic practice based in Rotherham. Our team consists of three GP partners, 5 Salaried GP's, Advanced Clinical Practitioner, Paediatric Advanced Clinical Practitioners, Paramedics, Nurse Practitioners, Practice Nurse, Nurse Associates, HCA's supported by a highly efficient administrative team.

As part of Health Village & Dearne PCN we work closely with a range of additional roles, including clinical pharmacists, mental health practitioners and care coordinators. Our clinicians balance independent practice with a strong collaborative approach, ensuring seamless integration of care across the practice and the wider network.

Job responsibilities

Primary KeyResponsibilities

Thefollowing are the core responsibilities of the Practice Nurse. There may be, onoccasion, a requirement to carry out other tasks. This will be dependent uponfactors such as workload and staffing levels:

a. To be instrumental in the support of the Infection Prevention Control(IPC) lead and to work to the IPC requirements by providing audits, training,support and evidence of compliance to the practice manager

b. To support in the publication of the IPC annual statement in conjunctionwith the practice manager

c. To develop, implement and embed an effective care management programmefor the frail and housebound patients registered at the practice

d. To assess the needs of patients ensuring the provision of healthcare isappropriate, incorporating evidence-based practice

e. To develop, implement and embed health promotion and wellbeingprogrammes

f. To implement and evaluate individual treatment plans for chronic diseasepatients that promote health and wellbeing

g. To identify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to patients health

h. As required, to provide routine nursing care to patients in accordancewith clinical based evidence, NICE and the NSF

i. As required, to diagnose and manage acute and chronic conditions

j. To review patient medications to enhance compliance

k. To provide guidance to patients in the use of prescribed andover-the-counter medications regarding side effects and dosages

l. To request pathology services and process pathology results asrequired

m. To oversee the provision of chronic disease clinics, delivering patientcare as necessary, referring patients to secondary/specialist care asrequired

n. To maintain accurate clinical records in conjunction with extantlegislation

o. To ensure SNOMED CT codes are used effectively by all members of thenursing team

p. To ensure the appropriate maintenance and use of chronic diseaseregisters

q. To develop, implement, embed and participate in well woman clinics

r. To develop, implement, embed and participate in well man clinics

s. To ensure compliance with the practice chaperone policy

t. To assist GPs with minor surgery when required

u. To prioritise health issues and intervene appropriately

v. To support the team in dealing with clinical emergencies

w. To recognise, assess and refer patients presenting with mental healthneeds

x. To implement vaccination programmes for adults and children

y. To support patients in the use of their prescribed medicines orover-the-counter medicines (within own scope of practice)

z. To liaise with external services/agencies to ensure the patient issupported appropriately (vulnerable patients etc.)

aa. To delegate clinical responsibilities appropriately (ensuring safepractice and the task is within the scope of practice of the individual)

bb. To support the clinical team with all safeguarding matters, inaccordance with local and national policies

cc. To understand practice and local policies for substance abuse andaddictive behaviour, referring patients appropriately

dd. To deliver opportunistic health promotion where appropriate

Secondary Key responsibilities

In additionto the primary key responsibilities, the Practice Nurse may be requested to:

a.Develop and review auditprotocols and process for the practice, effectively utilising the auditcycle

b.Develop, review and implementnursing protocols in conjunction with the partners

c.Support all members of thenursing team, providing guidance when necessary, conducting appraisals andtraining needs analyses as required

d.Participate in local initiativesto enhance service delivery and patient care

e.Support and participate inshared learning within the practice in order to improve patient care

f.Continually review clinicalpractices, responding to national policies and initiatives where appropriate

g.Participate in the review ofsignificant and near-miss events applying a structured approach, i.e., rootcause analysis (RCA)

h.Coordinate the nursing teamrota, ensuring sufficient staff are available to meet patient needs

i.Lead the planning andimplementation of changes within the nursing team, enhancing the provision ofservices to patients

j.Support the practice team withthe reviewing and implementation of practice policies and protocols, ensuringconformance to extant legislation


Person Specification


Qualifications

* Registered Nurse (RGN)
* Post Graduate Diploma or degree (Diabetes or Chronic Disease Management)
* Non-Medical Prescribing
* Mentor or Teaching Qualification


Experience

* Experience of working in a Primary Care environment
* Experience of working autonomously
* Experience of working with patients with diabetes
* Experience of working in a GP practice
