We’re looking for an organised and compassionate Dementia 1-1 Coordinator to join our Dementia Services team at Dougie Mac. This is an exciting opportunity to support people living with dementia across North Staffordshire and Stoke-on-Trent.
You’ll play a vital role in coordinating our 1-1 Dementia Support service, pairing Dementia Support Workers with individuals to engage in meaningful social activities and enhance wellbeing, while also providing much-needed carer respite.
Key Responsibilities
* Coordinate new referrals to the 1-1 Dementia Support service, acting as the first point of contact for service users and carers.
* Allocate Dementia Support Workers to service users and create personalised plans of care.
* Undertake assessments of new and current service users to ensure suitability for the service.
* Create and manage rotas for the 1-1 service, covering staff absence and ensuring continuity of care.
* Maintain effective communication with service users, carers, and relevant agencies to ensure high-quality support.
* Monitor and oversee relationships between service users and Support Workers, resolving issues as needed.
* Provide feedback and reports to team leaders to support quality improvement.
About You
You’ll be a proactive and empathetic individual with excellent organisational skills, passionate about supporting people with dementia and their carers. You’ll bring strong communication and coordination skills to this rewarding role.
We’re looking for someone who has:
* NVQ/QCF Level 3 in Health and Social Care or equivalent experience.
* Experience working in a health or social care setting with people affected by dementia.
* Knowledge of person-centred approaches to care and assessment processes.
* Strong interpersonal skills to build relationships with service users, carers, and teams.
* Excellent IT and administrative skills, including proficiency in Excel.
* A flexible and proactive approach, with the ability to work independently and as part of a team.
* A full UK driving licence and daily access to a vehicle.

Salary: £20,286.96 actual salary (FTE £25,358.70)
Hours: 30 hours (hours to be agreed, Dementia Services operates Mon – Friday)
