Job description

To work within a highly motivated team who provides support to children, young people, their families and carers through the Strength 2 Strength Programme across the Western Trust Area. To deliver an evidence-based programme to children and young people aged 8 13 years. Our interventions are based on a whole family approach which recognises and builds upon the families existing strengths and supports and the development of new skills and coping strategies. Through our work we aim to: - Support families to succeed and stay together - Improve school attendance rates and access to education or employment opportunities - Develop and improve emotional well being. - Increase parental capacity - Increase community involvement and social inclusion


Responsibilities

* In conjunction with line manager review and plan for new referrals including completion of initial assessment of service user.
* Contribute to the development of appropriate service user plans e.g. support plan, risk management plans.
* Ensuring all aspects of case management are undertaken including intervention planning and recording of all relevant information in line with Extern policy and legal requirements.
* Facilitate and participate in all aspects of planned interventions with service users (e.g. groupwork, overnight respite, individual work & activities).
* Assist in the development of service users risk assessments and support in continuous review as and when required.
* Follow necessary risk management requirements and procedures e.g. child protection, medication administration, lone working and on call procedures.
* Adhere to EXTERN Child and Vulnerable Adult Safeguarding procedures and relevant external standards.
* Attend and participate in internal and external meetings as required and act as advocate for services user when appropriate, team meetings, multi-disciplinary meetings, community hub meetings etc.
* Develop and maintain appropriate professional relationships with Service Users, colleagues and stakeholders. When required, liaise with appropriate relatives.
* Ensure compliance with necessary recording requirements and take responsibility for accurate and timely completion e.g. Extern’s internal database, service users files, SPPG monitoring and statistical data, evaluative information etc.
* Assist in the induction, instruction, supervision and training of new staff, bank workers and volunteers as appropriate.
* Responsibility for keeping abreast of current legislation and Extern’s policies and procedures. Attend appropriate training as and when directed by the Team Leader.
* Adhere to Extern’s Health & Safety Policies and Procedures e.g. Lone Working Policy, People safe, Medication and Transport policies.
* Adhere to safeguarding procedures and relevant internal and external policies and standards at all times. Report safeguarding concerns, in a timely manner, in line with Extern policy. Attend mandatory safeguarding training.


Skills and Qualifications

Essential Skills

* Minimum of a Level 3 Qualification (as set out in the RQF Framework) in a relevant field/subject i.e. for example, Youth Work, Social Care, Health & Social Care, Childcare, Parenting.
* In the absence of the qualification requirement, 2 years recent, relevant experience of working directly with a similar service user group (up to and including Hardiker Level 4).
* 6 months substantial experience of working within a social care setting dealing with young people with complex needs and challenging behaviours. I.T. competent, including use of MS Word, Excel and PowerPoint.
* Be flexible and willing to work in the evenings and at weekends when the needs of the service users dictate.
* Full driving licence and access to a car (please note relevant business insurance will be required on commencement of employment).
* Commencement of employment (you will be required to provide evidence of NISCC registration).

Work time: 40 standard hours (including breaks) (Flexibility Required)
