Liquid Personnel is currently recruiting for experienced and qualified Social Worker to join a busy Hospital Discharge Team based in Lewisham.
To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.

Essential requirements for the role:
5. Knowledge on the CHC process and discharge to assess pathways
6. Skilled report writing and manage the recording and administration around assessment and planning.
7. Identify and carry out Mental Capacity Assessments and chair best interest meetings.
Key Responsibilities:
8. To support vulnerable adults and to make informed decisions and choices in their use of resources, support and services.
9. Complete initial visits within 24 hours to ensure the citizen is is safe and then decide with pathway the patient should go.
10. To undertake assessments and then and re-assessments in line with the care Act 2014.
What we are looking for in a Social Worker:
11. Social Work England registration
12. Eligible to work in the UK
13. Hold a full UK license
14. Post-qualified experience
15. Degree level or equivalent in Social Work
