We’re looking for a compassionate and confident Care Liaison Advisors to coordinate ongoing care for patients and their families.
In this vital frontline role, you’ll be supporting individuals throughout their healthcare journey—answering queries, scheduling appointments, and ensuring every interaction is handled with empathy, efficiency, and care.
You’ll be part of a dedicated team that puts people first and works together to create a smooth, reassuring experience for all.

Every day you will…
1. Communication through regular touch points both over the phone and in person and updating system notes to accelerate patient discharge
2. Drive customer satisfaction and deliver results through execution of effective placements
3. Consistent and regular communication acting as the link between discharge teams, local authority, Next of Kin/Families, clients, ward nurses, care providers and service users to facilitate discharge and ensure a seamlessly coordinated service.
4. Ability to effectively organise caseloads In priority order having fully understood patient needs.
5. Flexibility when approaching all situations with a willingness to learn and an inquisitive mind.
