We are offering an exciting opportunity for a committed and compassionate Registered Nurse to join our innovative Frailty Same Day Emergency Care (SDEC) team on a full-time, fixed-term basis.
Working 37.5 hours per week working across 7 days from 08:30-18:30 - 4 days per week
We are offering an exciting opportunity for a passionate and committed Registered Nurse with ideally a background in Emergency care, or SDEC experience to join the Frailty SDEC team on a secondment ending the 31st of March 2026. This full-time role is ideal for a nurse seeking a role which requires flexibility in a fast paced, same day emergency care environment.
This is a rewarding role for nurses who are passionate about delivering holistic, patient-centred care in a fast-paced environment. Whether you're already experienced in emergency care or looking to gain valuable experience in SDEC, we would love to hear from you.
Main duties of the job
As a key member of the Frailty SDEC team, you’ll provide high-quality nursing care to older adults presenting with frailty syndromes. You’ll work closely with Emergency Department colleagues to proactively identify and manage patients, contributing to comprehensive geriatric assessments and the creation of supportive management plans.
You Will Be Responsible For
* The assessment, planning, implementation, and evaluation of nursing care
* Supporting shift coordination to ensure safe, efficient, and effective care delivery
* Collaborating with multidisciplinary colleagues including geriatricians, GPs, physiotherapists, and pharmacists
* Acting as a key link between primary, secondary and social care to reduce unnecessary admissions and support safe discharge planning
You will help promote admission avoidance, reduce hospital length of stay, and support early intervention for individuals living with frailty.

We’re seeking a nurse who:
* Is a Registered Nurse (NMC registered) with clinical experience—preferably in emergency care or SDEC
* Is confident managing complex patient needs and developing individualised care plans
* Demonstrates excellent communication, teamwork, and organisational skills
* Can work autonomously while contributing to a wider multidisciplinary team
* Is committed to delivering evidence-based, person-centred care and continuous service improvement
Training and support will be provided to help you grow and succeed in this role.

Qualifications
Essential criteria
* Registered General Nurse
* Current NMC registration
Desirable criteria
* Teaching certificate, SLIP, mentorship preparation for healthcare
Exoerience
Essential criteria
* Experience in caring for a variety of acute medical conditions
* previous experience triaging emergency and acute medical patients
* Experience in older people medicine
Desirable criteria
* Emergency care or SDEC experience preferred, but not essential
Skills and Knowledge
Essential criteria
* awareness of current professional nursing issues
* Ability to manage change within the working environment
* Good interpersonal skills with the ability to manage difficult/stressful situations
* Ability to train and supervise junior staff

* Ability to manage same day emergency care
