We support individuals in their own homes, assisting those with a variety of health needs, including learning disabilities, mental health issues, autism, physical disabilities, and those requiring assistance with personal care. The person you will be supporting has a diagnosis of Autism, learning disability and complex care needs. They have a bubbly personality and benefit from support staff who show initiative, can work independently, and collaborate well within a team. They enjoy sensory play, water-based activities, and accessing the community for outings such as meals, bowling, and trips to the cinema. Supporting individuals to live independently: Assist individuals in maintaining their independence by encouraging and supporting them in daily activities such as meal preparation, budgeting, and personal care. Encourage and support positive relationships with family, friends, and the wider community. Supporting with day-to-day activities: Provide practical and emotional support for individuals in areas such as shopping, appointments, social activities, and hobbies. Accessing the community: Assist individuals in accessing community resources and activities to enhance their social inclusion and well-being. Supporting outside the home setting: On occasion, you may be required to accompany individuals outside of their home for activities such as hospital visits, holidays, or attending appointments at police stations. Ensure each individual is supported in a way that respects their personal preferences, choices, and promotes their independence. Regularly report on the progress of the individuals you support and ensure their care plans are followed. Role Type: Full Time Hours: 36 hours (minimum) per week. Shift Patterns: Waking nights, 12 hours. Rochdale (Shawclough) Desired Skills and Qualifications: A caring and compassionate attitude, with a genuine desire to help others. 2 years supported living experience. Knowledge of person-centred care principles. Flexibility and willingness to provide support outside of the home setting when required. NVQ Level 2 or 3 in Health and Social Care (or equivalent) is desirable. Alternative weekend availability. Experience managing complex care needs and behaviours the challenge.
