Young People Leaving Care Outreach Support Worker.
Position Details:
* Outreach hours are between 8am and 8pm, Monday to Friday.
* This is a long-term temporary position with no fixed end date; quick start possible through a temp-to-permanent arrangement if subscribed to the update service and with a short notice period.
* Coverage areas include Sheffield, Leeds, and Hull.

Candidate Requirements:
* Experience supporting young people and their families.
* Full UK driving license with business insurance is essential.
* Experience working in similar settings; ideally NVQ Level 3 Children & Young People Workforce or equivalent.
* Willingness to undertake further training.
Key Responsibilities:
* Support young people in developing independence skills.
* Complete property inspections.
* Manage diaries and work closely with permanent staff.
* Assist in welfare and NHS issues, prepare reports, and arrange appointments with partner agencies.
* Provide transport/escort as directed, conduct inductions, and assist in move-on processes.
* Record and maintain electronic records accurately.
* Attend meetings, provide mediation, and use IT packages effectively.
* Work cooperatively across multidisciplinary teams, maintain confidentiality, and participate in policy development.
* Report housing issues, undertake on-call duties, and assist with reception duties as required.
Qualifications:
* High standard of general education.
* Preferably NVQ Level 3 in Health & Social Care – Children and Young People or similar.

Duration: Approximately 11 months. A quick start is possible with a current online DBS check.
