The Scottish Government has guaranteed a year’s post diagnostic support for everyone diagnosed with dementia.
What is post diagnostic support?
This support is delivered by a Post Diagnostic Support Link Worker, who is allocated to you for at least one year.
Following your diagnosis, and with your consent, a Link Worker will visit you to speak to you about your diagnosis, find out what matters to you and what kind of support is best for you.
This can include helping you to:
* understand the illness and manage your symptoms
* be supported to keep up your community connections and make new ones
* have the chance to meet other people with dementia and their partners and families
* plan for your future support.
