What you will do:
* Conduct comprehensive strength-based assessments in line with the Care Act 2014, Mental Capacity Act 2005, and other statutory requirements.
* Develop and implement personalised, outcome-focused care and support plans that are regularly reviewed.
* Lead safeguarding enquiries under Section 42 of the Care Act and contribute to crafting effective safeguarding plans.
* Foster independence and well-being by connecting service users with community resources, services, and support networks.
* Collaborate with health professionals, housing services, voluntary organisations, carers, and families to deliver cohesive care.
* Facilitate seamless hospital discharge planning and assist in transitions into community-based care.
* Perform Mental Capacity Assessments and act in the best interests of service users when necessary.
* Maintain precise and current case records in accordance with professional and council standards.
* Engage actively in team meetings, supervision, and continuous professional development.
* Participate in the development and delivery of services within the Adults Community Social Work Team.
