Discharge and Flow Team - Empowering Independence
Reablement is a short-term, therapist-led service focused on promoting independence and helping individuals break free from ongoing services. Our team provides information, needs analysis, equipment, adaptations, and targeted support.
Since March 2020, we have partnered with health services to transform hospital discharges in Wiltshire. The Wiltshire Locality Hub manages discharges seven days a week from acute hospitals and other facilities beyond the county borders.
This role offers variety and flexibility within our small team, where colleagues support each other to complete tasks.
As a Locality Coordinator, you will be central to our Wiltshire Locality Hub, coordinating hospital discharges alongside NHS and health partners, working across three main acute hospitals. The service operates seven days a week, and weekend work is required.
Applicants with backgrounds in health or social care, especially those experienced in supporting adults in their homes, are encouraged to apply. Knowledge of adult care legislation, safeguarding, and social care transformation is essential.

Job responsibilities
The Locality Coordinator will work in accordance with the Workforce Quality Assurance Framework, within a dedicated team, ensuring compliance with policies, standards, and procedures. The role involves triaging incoming work, coordinating safe hospital discharges, managing referrals, and ensuring effective community reablement services for a population of approximately 160,000 residents and a customer base of 1,000-1,500.
Specific duties and responsibilities
* Review customer situations, assess risks, and coordinate discharge plans.
* Identify eligible needs and outcomes, including prevention services.
* Collaborate with customers and agencies to develop support and discharge plans.
* Coordinate hospital discharges, acting as a link between providers and the community.
* Manage a caseload, working closely with occupational therapists to facilitate discharges, prioritizing tasks effectively.
* Support the Locality team and Reablement service, including data collection and reporting.
* Apply a person-centred approach, adapting to individual needs and circumstances.
* Provide updates and recommendations to occupational therapists.
* Develop understanding of risk assessment and positive risk-taking, referring complex issues to occupational therapists.
* Arrange equipment via Medequip to promote independence.
* Utilize community and voluntary resources to reduce long-term funded service needs.
* Consider family context in assessments and refer to additional services as needed.
* Advise customers on financial eligibility, referring to finance and benefits teams.
* Accurately record contacts, adhere to data protection, and ensure confidentiality.
* Update discharge information on the Adult Services system and use SystemOne responsibly.
* Facilitate integrated working with health teams, hospitals, and GP surgeries.
* Maintain effective communication with all stakeholders.
* Follow safeguarding policies, including making referrals and participating in investigations.
* Engage in supervision, training, and appraisal, committing to ongoing development.
* Follow complaints procedures and represent the Council professionally at all times.
